Sign Up Box Redirecting to Bizarre CTA

So if you enter an email address on my form on my Free Updates page (http://www.athomeweightlosscamp.com/rej ... -a-150.asp) a bizarre call to action comes up after you click submit. (See the image attached). The same is also happening when you use Sign Up Box #6 (the one I use at the end of all my blog posts). Instead this page should appear after the Submit button is clicked: http://www.athomeweightlosscamp.com/rej ... u--a-4.asp

This actually a setting per Signup Box, I looked and currently those two signup boxes are set to the Thank You Page of: Options Page. To have them go where is sounds like you want you must set this to be: Signup Thank You Page
